Just take any PDF, a format very close to the heart of the OS X operating system, and change it's extension to ".icns", this will make that PDF file's icon change to the contents of the file (the first page for a multipage file), and if it's an icon sized image, you're all set. Of course, it may be a bit fudged if you're PDF wasn't the exact dimensions. So, make that PDF the exact dimensions of the icon you'll be needing in Page Setup, and repeat the above instructions. You can also directly copy PDF and ICNS content (stuff from inside files of those types) onto the Get Info icon. What's curious is that Preview will not allow you to save a PDF as an ICNS.
